Print the Form and fill in each item. Be sure to include the children's names.

If you are paying by check, please mail the form and your check to MFOTL, P.O. Box 452, Moorpark, CA 93020, or drop it (or cash) off at the circulation desk at the Library.

If you are paying by credit card or Paypal, print and fill in the form and then return to this page and click on the Paypal button below to process your payment. Please note that at this time this feature is only available via standard computers, not via phones or iPads.

Print your credit card or Paypal receipt and attach it to your Letters from Santa Order Form(s) and mail to the address above. You may also scan the Order Form and your receipt and email both to mfotlibrary@gmail.com.
